This can occur when the fluid in a system has dropped below it's freezing temperature. Perhaps too much heat is being removed from the network via a heat exchanger? What you can consider here is adding a bypass loop at the effected heat exchanger and fit an orifice plate to the bypass until flow through Heat exchanger is maintained at a minimum level to prevent freezing occurring in pipe after it.
